COSL Drilling - COSLInnovator - Investigation of incident with fatal consequencesPublished:The PSA has completed its investigation into the incident on 30 December 2015 when mobile offshore unit COSLInnovator was struck by a wave. One person was killed and four injured, while the unit suffered extensive damage to part of its living quarters.

Statoil - Gudrun - Investigation into hydrocarbon leakPublished:The PSA has completed its investigation into a hydrocarbon leak on the Gudrun facility in the North Sea on 18 February 2015. Serious breaches of the regulations have been identified, and the PSA has now given operator Statoil notification of an order.
