The Petroleum Safety Authority Norway's investigation of the gas leak at the Kårstø facility in January 2016 has concluded. The investigation identified a number of regulatory breaches.

The PSA has completed its investigation into the incident on 30 December 2015 when mobile offshore unit COSLInnovator was struck by a wave. One person was killed and four injured, while the unit suffered extensive damage to part of its living quarters.

The Petroleum Safety Authority Norway's investigation of the well-control incident at Visund A in March 2016 is now complete. The investigation identified several breaches of the regulations.

The PSA has completed its investigation into a hydrocarbon leak on the Gudrun facility in the North Sea on 18 February 2015. Serious breaches of the regulations have been identified, and the PSA has now given operator Statoil notification of an order.

A 31-kilogram grating plate fell eight metres on the Heidrun facility on 22 September 2015, grazing a person who suffered pain and swelling in one shoulder. The PSA’s investigation of the incident has identified several nonconformities from the regulation

The PSA has completed its investigation into Norske Shell and contractors' own follow-up of incidents involving falling objects at Nyhamna in 2015. No non-confirmities with the regulations were identified in connection with our investigation.

The Petroleum Safety Authority Norway (PSA) has completed its investigation of a fire on Petrojarl Knarr during 24 March 2015. A number of breaches of the regulations have been identified.

The PSA has completed its investigation into two undesirable incidents which occurred on the Eldfisk complex on 6-8 August 2014. Several breaches of the regulations have been identified.

The Petroleum Safety Authority Norway has completed its investigation of an incident involving a dropped object on mobile drilling unit West Venture on 23 March 2015.

The Petroleum Safety Authority Norway has completed its investigation of an incident involving personal injury on Transocean Barents on 4 March 2015. Several nonconformities from the regulations have been identified.
