On Thursday, 15 February 2024, Havtil was notified by the operator Equinor of a short circuit with subsequent fire and smoke production in a substation for electrical power distribution at Mongstad.
The entire facility was routinely evacuated of personnel not critical to operations, and the processing facility was shut down. Nobody was harmed during the incident.
Shortly afterwards, Havtil decided to investigate the incident. Our investigation has now been completed.
Result
In connection with the investigation, we identified four non-conformities. These were:
- Deficient compliance with own requirements
- Deficiencies in the design of high voltage room
- Deficient prevention of failure modes, monitoring and control of failure mechanisms
- Deficiencies in the process of initiating projects at a facility in operation
We also identified two improvement points. These were:
- Evaluation of maintenance effectiveness
- Communication systems
Exempt from publication
The investigation report is exempt from public disclosure with reference to the Freedom of Information Act, Section 24(3). There are so many parts of the document that must be exempted that the entire document is exempt (FOI Act, Section 12(c)).
What happens now?
The investigation has been completed. We have asked Equinor to report on how the non-conformities will be addressed, and for their assessment of the improvement points observed.
