Objective

The objective of the audit was to follow up Equinor Mongstad’s compliance with regulatory requirements related to the prevention of hydrocarbon leaks, as well as measures implemented to reduce the risk of unwanted incidents. We also wanted to investigate how safety-critical functions and work processes are safeguarded and followed up and how Equinor ensures relevant learning across the organisation.

Furthermore, we wanted to look at barrier management, experience sharing and implementation of measures following selected incidents in the light of Equinor’s internal investigations and reviews. What changes or improvements have been made with regard to the establishment, design and follow-up of relevant safety functions and any associated work processes.

The audit’s objectives also included Equinor’s compliance with the Major Accident Regulations, for which this year’s theme concerning major accident supervision is risk mapping and risk analyses.

Result

Two non-conformities were identified concerning:

  • Inadequate measures and learning following incidents
  • Failure to prevent blockages in drainage pipes

In addition, one improvement point was identified related to inadequate compensatory measures in the event of the loss of the sight glass on a fuel gas container.

What happens next?

We have asked Equinor to report by 4 March 2026 on how the non-conformities will be addressed and for their assessment of the improvement point observed.
