The audit was conducted from 21 to 31 October 2025.

Objective

The objective of the audit was to verify CDE’s compliance with the HSE regulations relating to stability, watertight and weathertight integrity, ballast systems and wave-in-deck impacts on COSLInnovator.

Result

17 non-conformities were identified concerning:

  • Inadequate specification of operational limitations
  • Daily monitoring of stability
  • Maintenance of weathertight closures
  • Plan for closing weathertight closures
  • Maintenance of exterior watertight sliding doors
  • Indication of watertight hatches
  • Watertight integrity panel
  • Watertight HVAC valves
  • Indication of position of watertight HVAC valves
  • Failing watertight HVAC valve
  • Overview of watertight-barrier weaknesses
  • Procedure for ballast-system operation
  • Failure mode for ballast valves
  • Ballast operation at secondary level
  • Indication of ballast valves’ position
  • Procedure for operating ballast valves at secondary level
  • Deficient marking of ballast-system components

In addition, six improvement points were identified concerning:

  • Maintenance of interior watertight sliding doors
  • Lack of training in damage simulation and consequent recovery
  • Procedure for post-damage recovery
  • Limitations in temporary condition
  • Freeboard plan

What happens next?

We have asked COSL to report to us by 30 January 2026 on how the non-conformities will be addressed, and to give us their assessment of the improvement points observed.
