Background
On 31 January 2024, an incident occurred on Equinor's Åsgard A facility, whereby a cabin window was stove in by green water.
Havtil investigated the incident.

Havtil’s investigation identified non-conformities relating to inadequate green water assessments, lack of compliance with internal requirements for covering of cabin windows and inadequate utilisation of experiential knowledge, as well as weakened fastening capacity of cabin windows.
On this basis, we issued an order to Equinor requiring the company to identify measures and establish a plan for their implementation to reduce the likelihood of damage, failure, hazard and accident situations occurring as a result of green water incidents, and to identify measures and establish a plan for their implementation to ensure that experiential knowledge from their own operations is used in the improvement work.
Equinor’s own investigation identified recommendations for learning and improvement needs aimed at the facility, and across Equinor. The recommendations were assessed by a separate working group that created a learning plan. The order and non-conformities have been followed up through the measures in the learning plan.
Objective
The aim of the audit was to verify that the implementation of measures reduces risk related to green water incidents and contributes to learning. In the audit, we verified whether non-conformities and the order have been addressed in line with the response in Equinor's letter of 28 February 2025.
Result
The audit identified no non-conformities or improvement points.
