The audit was conducted between 13 April and 7 May 2021.

Objective

The PSA’s letter of allocation for 2021 states that the prevention of major accidents is a prerequisite for operating in a prudent manner and is a fundamental element of all phases of the petroleum activities. It also states that the identification and management of safety-critical factors that affect major accident risk and the choice of solutions are crucial for ensuring prudent operations.

The intention of this audit was to verify that Equinor chooses solutions and identifies and manages safety-critical factors in such a way as to reduce the risk of major accidents.

Result

Two non-conformities were identified concerning:

  • Inadequate staffing and competence
  • Deficient follow-up
