Background

From 5 to 7 September 2017 we carried out a major accident audit of Statoil Mongstad. The theme of this audit was the maintenance of safety-critical equipment, with a special focus on safety-critical valves.

The audit took place at Statoil's refinery at Mongstad in Hordaland.

The audit pursuant to the regulations on measures to prevent and limit consequences of major accidents in enterprises where hazardous chemicals are present – the Major Accident Regulations – was coordinated by KFS, the coordinating group for these regulations. In accordance with KFS guidelines, a major accident audit of Mongstad is to be conducted annually.

Objective

The objective of the major accident audit was to verify that Statoil is fulfilling the regulatory requirements at Mongstad, including the Major Accident Regulations and the guidelines from KFS, which for 2017 concern maintenance.

Result

The audit identified one regulatory non-conformity. This concerned risk assessments when suspending preventive maintenance. We also observed two improvement points. These related to the categorisation of safety-critical valves and the use of empirical data.

We have asked Statoil to report on how the non-conformity will be addressed by 12 December 2017.
