One person lost their life in a workplace accident at the Mongstad facility on 17 September 2025.

The accident is reported to have occurred during a lifting operation.

Investigation

Havtil has decided to initiate its own investigation into the incident. Our specialists are now beginning this work.

The main objective of the investigation is to determine the causes of the incident and any lessons to be learned, and share this information with the industry.

The investigation will include both Equinor, the operator of the Mongstad facility, and the supplier Crane Norway.

As part of the investigation, we will:

  • Thoroughly review the sequence of events
  • Identify both immediate and underlying causes
  • Use necessary sanctions to correct any breaches of regulations
  • Present the investigation findings to the public
  • Contribute to the transfer of experience and lessons learned to other industry participants.

The investigation will be summarised in a report to be published on havtil.no.
