Equinor has established a number of HSE systems for identifying and securing an overview of the condition and integrity of the facilities. From 13 to 16 May 2019, we carried out an audit of Equinor’s use of these systems.

Objective

The objective of the audit was to monitor that the HSE systems in use for ensuring safe operations are being employed in conformity with the regulatory requirements. The main focus of the audit was the company’s use of the TIMP, Synergi, SAP and A-10/Technical Condition Report systems, as well as routines and practices for alerting and notifying us of hazard and accident situations. During the audit, we also looked at whether the systems were used consistently across the different facilities and operating areas in the north and south.

Result

The audit identified one non-conformity and four improvement points.

Non-conformity:

  • Follow-up of barrier valves

Improvement points:

  • Handling of uncertainty in grading in TIMP
  • Weaknesses in mapping the ability of technical barrier elements to perform their roles in an accident situation (survival)
  • Deficient monitoring that established control systems within risk management work as intended
  • Internal audit of HSE systems

What happens now?

We have asked Equinor to report on how the non-conformity will be addressed. We have also asked the company for an assessment of the improvement points we observed.
